The latest five subs vote at the meeting of the 20 “shareholder” clubs was deadlocked at 10 votes to 10.
That was well short of the 14 -6 majority required under Prem regulations for a rule to be changed.
The decision came despite lobbying from PFA boss Gordon Taylor and the urging from FA chief executive Mark Bullingham for a change of heart.
And it also failed even though 15 of the 20 top flight bosses having signalled their support fo a change at a meeting last month.
SunSport previously revealed that the move was set to be backed by top bosses led by Jurgen Klopp and Pep Guardiola.
Yet English football’s top flight is now left as the only major league in Europe to stick with three substitutes.
All of Europe’s other Big Five leagues – Germany, France, Italy and Spain – stuck with the five substitute rules introduced under a “temporary” change by football’s Law-making body Ifab in July.
Uefa initially reverted to three subs for European and international competition.
It has now reversed position under pressure from players’ chiefs, with the EFL changing its position last month and the FA agreeing to allow five subs in the FA Cup from the third round stage next month.
However, a separate vote to extend bench options to nine players rather than the current seven, did go through.
That measure was approved by 16 votes to four and is likely to come into effect from as soon as this weekend.
