The deal was completed on Tuesday evening before Portugal’s transfer window shut at midnight.
Serbian international Grujic spent the last two seasons on loan at Hertha Berlin but was permitted to join Porto after Liverpool bosses turned down permanent bids from the Bundesliga.
Midfielder Grujic will join up with the Portuguese champions after the international break but there is no option to buy included in the loan agreement.
West Ham’s former record signing Felipe Anderson, 27, is also making the move to the Estadio do Dragao.
Anderson joined the Hammers for £35million in 2018 but has slipped down David Moyes’ pecking order – only starting three Carabao Cup matches this term.
After bagging 12 goals and as many assists in 72 appearances, Brazilian midfielder Anderson has been allowed to make the loan switch abroad to the Primeira Liga.
Porto also welcomed Chelsea’s Malang Sarr to complete their Premier League triple swoop.
Sarr only joined The Blues this summer from Nice but is earmarked as a star of the future.

The French defender, 21, moved to Stamford Bridge on a free just five weeks ago.
But after selling left-back Alex Telles to Manchester United, Porto have decided to strengthen their backline.
Sarr is one of 28 Chelsea players out on loan after Marco van Ginkel, George McEachran and Juan Castillo were also shipped out.
