Leading the line with Antoine Griezmann, Carles Perez put the Spanish champions in front on his Champions League debut after just 23 minutes.
But ex-Manchester United striker Romelu Lukaku levelled the scores on the stroke of half-time.
Inter striker Lautaro Martinez had the ball in the net twice after the break, only for both goals to be correctly disallowed for offside.
However, the night belonged to Fati as he smashed in a sublime winner inside the final five minutes at the San Siro.
The forward’s goal makes him the youngest player to score in the Champions League at 17 years and 40 days.
The Italian side needed to at least match Borussia Dortmund’s result against Slavia Prague to maintain second place.
But the German giants won 2-1 to move into the runner-up spot and consign Inter to a Europa League place.
Barca top the group with 14 points ahead of Dortmund on 10, while Inter finish third on seven ahead of rock-bottom Slavia Prague on two.
Fati netted his third of the season having made 12 appearances.
The La Liga leaders acted fast to tie down the teen starlet for another three-years, with his release clause set to £144million.
But it will increase to an incredible £339MILLION when the forward eventually signs a pro contract as a full-time member of the squad.
